2008-2009 Student Application Form
Dear Parents/Guardians,
Choosing a school for your child is one of the most important decisions you can make. Today
in New Orleans, you have more choices than ever. This is a common application form that
can be used to apply for acceptance at most of the city’s public schools, including tradi-
tional public schools and public charter schools.
Be sure to completely fill out the form and submit a copy to each school where you’d like to
apply. We encourage you to apply for several schools. You must submit a form for each
child.
This is an application form, not a registration form.
Submission of this application form does not guarantee your child will be accepted. Be sure
to ask the school about its admission policies. If you do not get into any of the schools where
you applied, contact the Recovery School District at (504) 373-6200. If you are a resident of
New Orleans, your child is guaranteed a place in a public school in New Orleans.

NSTRUCTIONS

Decide which schools you would like to apply to for your child. You
can apply to more than one school, but you can only REGISTER at
one.
∗
Review the admission policies of each school you are applying to
(by visiting or calling the school).
∗
Complete this application form. You must fill out a form for each
child attending school.
∗
Submit a copy of this application form directly to the schools
where you wish to apply, to the Recovery School District Welcome
School, 2239 Poydras Street or to the New Orleans Public Schools
Central Office, 3520 General DeGaulle Drive.
∗
Some schools may use a lottery if demand exceeds available slots.
∗
You will be contacted the week of March 10 by each school
where your child has been accepted.
∗
Once accepted at a school, you must go to the school to officially
register your child. Please contact the school about the docu-
ments you will need to bring to complete enrollment.
∗
You must officially enroll your child by March 28.
∗

1. You may apply to many schools, but you must complete an application form for each school and for each child. This is an
application, not a registration form. Some schools may require additional information, such as past grades, parent/student
interview, etc. Be sure to contact the schools where you are applying to ask about additional requirements.
2. Advancement to the 5th and 9th grades is dependent upon passing the LEAP exam.
3. Students in 4th and 8th grades who did not pass the LEAP exam will have the option of returning to their current schools.
